PT-141, also known as
bremelanotide (brand name Vyleesi), is a prescription peptide medication used to treat hypoactive sexual desire disorder (HSDD) in premenopausal women. It works by targeting the central nervous system to enhance sexual arousal and desire. 

 
What PT-141 Does

 Increases sexual desire (libido) in men and women

  • Enhances sexual arousal
  • Can improve erectile response even when blood-flow meds don’t work
  • Works independently of testosterone, estrogen, or dopamine 
     It acts on the MC3 & MC4 receptors in the hypothalamus

Men

Low libido (even with normal testosterone)

  • Erectile dysfunction (especially mental/arousal-related)
  • SSRI-induced sexual dysfunction

Women

 Hypoactive sexual desire disorder (HSDD)

  • Low arousal or mental sexual “disconnect”

    • Mechanism of Action: Unlike medications that increase blood flow to the genitals (like Viagra or Cialis), PT-141 activates melanocortin receptors in the brain's hypothalamus, which play a key role in regulating sexual desire.
  • FDA-Approved Use: It is only approved by the FDA for acquired, generalized HSDD in premenopausal women when the condition is not due to a medical problem, a mental health problem, relationship issues, or other medication use.
  • Off-Label Use: In men, it is sometimes used off-label for erectile dysfunction (ED) and low libido, and studies have shown it can be effective, particularly when combined with PDE-5 inhibitors like sildenafil. However, this use is not FDA-approved, and its long-term safety is less established for men.
  • Administration: It is typically administered as a subcutaneous injection into the abdomen or thigh at least 45 minutes before anticipated sexual activity. Some compounding pharmacies also offer it as a nasal spray, which may have a different side effect profile.
  • Dosage: No more than one dose is recommended per 24 hours, and no more than eight doses per month.
  • Common Side Effects: The most frequent side effects include nausea, flushing (a feeling of warmth and redness), headache, and injection site reactions.
